Trilogy Home Healthcare Jacksonville is hiring a dedicated Occupational Therapist to perform home health visits in St. Johns County, FL. You will assess patients, develop plans, and coordinate with doctors and the clinical team to improve mobility and independence.

Qualifications

• Graduate of an accredited OT program (AOTA-approved).
• Active Florida OT license.
• Valid Florida driver’s license, active vehicle insurance, and clear MVR.
• 1+ year OT experience required; home health experience preferred.
• Strong initiative and independent judgement.

Responsibilities

• Deliver high-quality, personalized OT care in the home setting.
