Occupational Therapist – Full Time

Sun West Choice Healthcare & Rehabilitation in Sun City West, AZ is seeking a full‑time occupational therapist to join our in‑house therapy team. Employment Details

Full Time: Flexible schedule with work‑life balance.

Experience & Qualifications

Licensed Occupational Therapist. This position is open to OTs at all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients per regulatory and clinical practice requirements.
• Administer procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record in accordance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and refer as appropriate.
• Provide consultation and counseling to patients, families, caregivers and other service providers regarding physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers and treatment plans.
• Provide comprehensive discharge summary of services per regulatory and clinical requirements.
