Job summary

Caringhand Home Health Solutions, Inc. is seeking an Occupational Therapist to help patients regain independence in their home environments. You will evaluate functional abilities, create treatment plans, and teach daily living skills in a field-based, per diem role requiring travel to patient homes.

Responsibilities include coordinating with the care team, documenting progress, and assisting with discharge planning. A valid CA OT license and CPR required; travel is necessary.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA/ AOTA
• Registered with the National Registration Examination of the AOTA
• Active California Occupational Therapy license and CPR certification

Responsibilities

• Evaluate patients’ functional abilities and home environments
• Develop personalized treatment plans and communicate with physicians and care team
• Teach and retrain patients in daily living activities
• Recommend adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with care team for seamless, patient-centered care
• Educate family and caregivers on treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning as patients transition from home health care

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $130 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
