Job summary

Cornerstone Rehab is seeking a full-time Occupational Therapist to deliver care to adult and geriatric patients on site at our Kentucky facilities. You will provide OT services, document progress, and participate in discharge planning while collaborating with a diverse clinical team.

We offer a sign-on bonus of $2,500 for full-time hires, access to medical/dental/vision insurance, disability coverage, life insurance, and a 401K plan. Travel to nearby centers is required as needed.

Qualifications

• Valid OT License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide occupational therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
