Job summary

Continental Springs in South Sioux City, NE is seeking a compassionate Occupational Therapist to join our interdisciplinary rehab team in a flexible part-time or per diem role.

You will evaluate residents, develop treatment plans, provide therapy to improve daily activities and mobility, and collaborate with PT, SLP, nursing, and physicians to support positive outcomes.

Qualifications

• Active license in Nebraska or eligibility to licensure.
• Graduate degree in Occupational Therapy from an accredited program.
• NBCOT certification preferred.
• New graduates and experienced therapists are welcome to apply.
• Experience in skilled nursing or subacute rehabilitation preferred.
• Strong communication and patient-centered skills.

Responsibilities

• Evaluate residents' functional abilities, including ADLs, mobility, cognition, coordination, and safety.
• Develop and implement individualized occupational therapy treatment plans.
• Provide therapeutic interventions to improve independence with daily activities and functional mobility.
• Address deficits related to strength, range of motion, balance, coordination, cognition, and fine motor skills.
• Recommend and train residents in the use of adaptive equipment and compensatory strategies.
• Educate residents, caregivers, and staff to promote safety, independence, and carryover of therapy goals.
• Collaborate with PT, SLP, nursing, physicians, and other members of the interdisciplinary team to support positive patient outcomes.
• Monitor resident progress and modify treatment plans as clinically appropriate.
• Maintain timely, accurate, and compliant clinical documentation.
• Participate in care planning and interdisciplinary team communication
