Job summary

Palms-Home-Care seeks an experienced Occupational Therapist to provide in-home health services focused on enhancing patient independence and safety within their home environment.

You will conduct in-home evaluations, develop personalized treatment plans, and assist with ADLs, adaptive equipment, and home safety modifications. OASIS documentation and collaboration with nursing/PT teams are required.

Qualifications

• Graduate OT program completion and licensure required.
• Home health experience preferred and familiarity with OASIS strongly preferred.
• Strong documentation and time management skills.
