Job summary

Diamond Care Group is expanding and seeking an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety within their home environment in Sarasota, FL. This role emphasizes compassionate, patient-centered care and collaboration with a multi-disciplinary team.

The ideal candidate will hold an active Florida OT license, have a background in home health, and possess strong documentation and time-management skills.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home evaluations and functional assessments
• Develop and implement individualized treatment plans
• Assist patients with ADLs (bathing, dressing, grooming, toileting)
• Train patients on adaptive equipment and home safety modifications
• Complete OASIS documentation as required
• Collaborate with nurses, PTs, and other team members
