Job summary

On Lok seeks an Occupational Therapist to provide OT treatment and interventions to our participants in the PACE program. You will supervise Rehabilitation Aides and guide families in the Functional Maintenance Program.

You will collaborate with an interdisciplinary team, manage administrative tasks, and support center operations. California OT licensure and NBCOT certification are required, with at least two years in geriatric care.

Qualifications

  • Graduate of an approved Occupational Therapy program.
  • Successful completion of NBCOT certification and initial registration.
  • Licensed in California through CBOT.
  • Two years experience with physical disabilities, geriatric focus.
  • Flexible, self-motivated and resourceful.
  • Ability to work independently and in an interdisciplinary team.
  • Physical ability to lift up to 75 lbs occasionally and 50 lbs frequently.
  • Ability to stoop, reach, stand, walk, push, pull and handle repetitive motions.

Responsibilities

  • Provides Occupational Therapy treatment and interventions to On Lok participants.

  • Participates in interdisciplinary team planning and problem-solving.

PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.
Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the administrative direction and supervision of the Center/Program Manager and the clinical supervision of the Occupational Therapy Program Specialist, the Occupational Therapist provides treatment &/or interventions and oversees the OT Functional Maintenance Program to On Lok participants.
Duties / Responsibilities

  • 1. Provides Occupational Therapy treatment & interventions to On Lok participants.
  • 2. Provides training, guidance, and supervision to Rehabilitation Aides and participant/family member in carrying out the Functional Maintenance Program.
  • 3. Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem solving, and processes.
  • 4. Fulfills all administrative requirements, including but not limited to center-based supervision of rehabilitation department in collaboration with PT, attendance at team & staff meetings, and recording & tracking of OT rehabilitation equipment.
  • 5. Other related job duties as assigned.
QUALIFICATIONS (knowledge, Skills, Abilities)
  • 1. Graduate of an approved Occupational Therapy program.
  • 2. Successful completion of certification exam & initial registration through National Board for Certification in Occupational Therapy (NBCOT).
  • 3. Licensed in the state of California through California Board of Occupational Therapy (CBOT).
  • 4. Two years experience in physical disabilities, with focus in working with the geriatric or frail population required.
  • 5. Flexible, self-motivated, and resourceful.
  • 6. Ability to work independently, and as part of an interdisciplinary team.
  • 7. Physical requirements include ability to lift/move up to 75 pounds occasionally, and up to 50 pounds frequently.
  • 8. Physical activities require the ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repetitive motions of the wrist, hands, and fingers.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.

Salary: $53.55 - $71.40 per hour
