Job summary

Caringhand Home Health Solutions, Inc. seeks an Occupational Therapist to help patients regain independence in their homes. This field-based, per diem role requires travel to patient residences and pays $130 per visit.

It offers an opportunity to impact daily living activities and work with a supportive care team. Responsibilities include evaluating safety, developing treatment plans, teaching daily living skills, and coordinating with physicians.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Registered with the National Registration Examination of the AOTA
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
