Montello Care Center in Wisconsin is seeking an Occupational Therapist to provide and administer occupational therapy to patients and residents under physician orders and the plan of care, with supervision by the Director of Clinical Services. This in-house therapy program supports consistent care and professional growth.

Qualifications

• Graduate of an accredited occupational therapy program.
• Licensed to practice in Wisconsin.
• License must not be excluded or sanctioned.
• Demonstrates professional attitude and appearance.
• Strong administrative and communication skills within department, facility, and company.
• Thorough knowledge of therapy principles, methods, and equipment.
• Familiarity with outpatient, ALF, ILF and/or LTC settings.
• Understanding of payer source documentation and requirements.

Responsibilities

• Uphold professional, legal, and ethical standards of occupational therapy as defined by AOTA.
• Manage service and operational excellence for all assigned rehab activities.
• Perform initial patient examination, evaluate referrals, and develop the initial plan of care for each patient.
• Create and maintain patient records for each patient treatment.
• Perform other duties as required and/or assigned.

Compensation:

$45.00 - $55.00 per hour, dependent on experience

Key Responsibilities:

Must uphold the professional, legaland ethical standards of practice for occupational therapy as defined by the AOTA and State Practice Act

Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes requiredto meet or exceed the expectations of those utilizingor impactedby the rehab dept

Performs initial patient examination, evaluation, interpretation of referrals, and creates the initialplan of care for each patient and revises as appropriate

Creates and maintainsa patient record for each patient treatment

Performs other duties as requiredand/or assigned

Qualifications:

Licensed in the state in which they will be practicing.

Must notbe excluded or sanctioned from state or federal program.

Demonstrate professional attitude and appearance.

Proficient administrative and communication within department and facilityand company.

Thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline.

Display working knowledge of multidisciplinary practice scopes in the Outpatient, ALF, ILF and/or LTC setting.

Thorough knowledge of all payer source documentation and payment requirements.

Ability to use analytical skills to develop outcome driven solutions.
