Job summary

Grand River Health in Rifle, CO is seeking a licensed Occupational Therapist for a full-time day shift in our Care Center. You will develop personalized treatment plans to maximize residents' independence and quality of life, collaborate with an inter-disciplinary team, and contribute to a community-focused facility with strong ratings.

Ideal candidates have an OT degree (Bachelors/Masters/Doctorate), CO license, and current BLS CPR certification; long-term care experience is preferred.

Qualifications

  • Must have a bachelor's, master's, or doctorate in occupational therapy from an AOTA-accredited program or be scheduled for the board examination.
  • Must hold current license in the State of CO.
  • Current BLS (CPR) certification at hire and maintained through GRHD employment.
  • Practical experience in all aspects of occupational therapy; long-term care experience preferred.
  • Ability to maintain good working relationships within the department and with other departments.

Responsibilities

  • Teach adaptive techniques and introduce equipment that supports participation in activities.
  • Monitor and document resident progress, adjusting treatment as needed.

What You’ll Do

The Occupational Therapist is a key part of our Care Center team, helping residents maximize independence and quality of life through personalized therapy plans. In our community-centered residence (built in 2020), you’ll work with no more than 54 residents in a supportive, above-average-rated facility that takes pride in providing exceptional care.

  • Develop and carry out individualized treatment plans to help residents improve or maintain daily living skills.
  • Teach adaptive techniques and introduce equipment that supports participation in activities.
  • Monitor and document resident progress, adjusting treatment as needed.
  • Provide therapy following evidence-based, traditional practices in line with Rehabilitation Services standards.
  • Deliver care in a long-term care setting, collaborating closely with an interdisciplinary team.

Who We’re Looking For
  • Must have obtained a bachelors, masters, or doctorate degree in occupational therapy at a college or university which has been accredited by the American Occupational Therapy Association, or be scheduled for the board examination.
  • Must have current license with State of CO.
  • CurrentBLS (CPR) certification upon initial hire and must maintain through GRHD employment.
  • Practical experience obtained from previous experience in all aspects of occupational therapy; working experience in a long term care setting preferred.
  • Ability to maintain good working relationships both within the department and with the other departments.
