Job summary

Interim HealthCare is seeking a Home Health Occupational Therapist (OT) in Michigan. Provide therapy in patients' homes, working with a multi-disciplinary team to meet personalized goals and enhance independence in daily activities.

Qualifications

• Graduate of an accredited Occupational Therapy Program and active OT license in Michigan.
• Minimum of 1 year of occupational therapy experience, ideally in home healthcare.
• CPR certification is required.
• Knowledge of state and federal home health regulations is preferred.
• Good clinical judgement and strong interpersonal skills are essential.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Collaborate with home health team (RN, LPN, CNA, HHA, PT, SLP) to follow the plan of care.
• Assess patient, observe deficits, set therapy goals and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment (grab bars, shower chairs) to improve safety.
• Assess fall risks and educate on home safety and plan of care with family.
