Job summary

Interim HealthCare of Northern Illinois is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. You will provide OT services within a patient-centered plan of care, coordinating with an interdisciplinary team including RNs, LPNs, CNAs, HHAs, PTs and SLPs.

The role emphasizes safety, personalized goals, and family education to improve daily living activities. A valid Illinois OT license and CPR certification are required, with home health experience

Qualifications

• Graduate of an accredited OT program and active OT license in Illinois.
• Minimum of 1 year of occupational therapy experience, ideally in home health care.
• CPR certification; knowledge of state and federal home health regulations.

Responsibilities

• Provide occupational therapy to homebound patients to improve daily tasks.
• Assess patient, observe deficits, set therapy goals, and document progress.
• Educate patient and family on plan of care, exercises, goals and self-care.
