Job summary

Interim HealthCare of Michigan is seeking a Home Health Occupational Therapist to deliver client-centered therapy in patients' homes. You will assess daily living activities, set goals, and collaborate with an interdisciplinary team to restore independence.

A Michigan OT license is required, CPR, and at least 1 year of experience in home health preferred. Part-time schedules (paid by visit), mileage reimbursement, and a comprehensive benefits package are offered.

Qualifications

• Graduate of an accredited OT program and active OT license in Michigan.
• Minimum of 1 year of OT experience, ideally in home healthcare.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks at home.
• Collaborate with an interdisciplinary home health team to align on goals.
• Assess and document patient progress, adjust goals as needed.
• Assist with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment and assess fall risks for home safety.
• Educate patient and family on the plan of care and self-care strategies.
