Job summary

Desert Peak Care Center in Phoenix, AZ is seeking a dedicated Occupational Therapist (OT) to join our interdisciplinary team. You will assess residents, design and implement therapy plans, and support mobility and daily living activities to promote independence.

This full-time role emphasizes collaboration with physicians and staff, documentation of progress, and ongoing quality improvement to enhance resident outcomes. A Nevada OT license is required to practice in this position.

Qualifications

  • Current licensure as an Occupational Therapist (OT) in the state of Nevada.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.

Responsibilities

  • Conduct comprehensive physical therapy assessments to identify residents' physical, cognitive, and emotional needs.
  • Develop and implement individualized therapy plans that focus on promoting functional abilities, independence, and overall well-being.
  • Provide physical therapy interventions, including cognitive training, sensory integration, and adaptive equipment training.
  • Collaborate with other healthcare professionals to ensure seamless care delivery and effective communication.
  • Maintain accurate and detailed records of therapy sessions, including progress notes and treatment plans.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided.

About the Role

We are seeking a highly motivated and compassionate Physical Therapist (PT) to join our interdisciplinary team. As a PT, you will work closely with our residents, physicians, and other healthcare professionals to develop and implement personalized therapy plans that promote independence, mobility, and overall well-being. Your expertise will enable our residents to maintain or improve their functional abilities and support their return to their highest level of daily living.
