Job summary

Pinehavencarecenter is seeking an Occupational Therapist to join our skilled nursing and rehabilitation facilities. You will evaluate residents, create personalized treatment plans, and work with the rehab team to restore independence in daily activities.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Valid Occupational Therapy license and registration for the state of practice.
• Experience in long-term care or geriatrics is preferred but not required.
• Strong communication and organizational skills.
• Compassionate, team-oriented, and committed to resident-centered care.

Responsibilities

• Evaluate referred residents to determine functional level and rehabilitation potential.
• Develop individualized treatment plans to help residents regain independence in ADLs.
• Establish treatment goals based on abilities, limitations, and physician recommendations.
• Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills.
• Educate residents and families on home programs and adaptive techniques.
• Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Collaborate with the interdisciplinary team to ensure a resident-centered approach to care.
• Adhere to applicable federal and state regulations, including HIPAA and Residents' Rights.
• Performs additional duties as assigned by the Director of Rehabilitation.
