Job summary

Trinity Healthcare Talent Services, LLC is seeking an Occupational Therapist to join a 220-bed Skilled Nursing Facility in Norfolk, VA. The role focuses on evaluating patients, designing individualized treatment plans, and delivering interventions to improve motor and daily living skills.

Responsibilities include interprofessional collaboration, progress monitoring, and educating patients and families for ongoing independence after discharge.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current state license as an Occupational Therapist.
• Minimum of 1 year of experience in a SNF or long term care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.

• Conduct patient evaluations to determine physical and cognitive abilities, limitations, and goals.
• Develop individualized treatment plans based on patients' needs and goals.
• Provide therapeutic interventions, exercises, and activities to help patients improve their motor skills, coordination, and daily living skills.
• Collaborate with other healthcare professionals, such as physicians and speech therapists, to ensure comprehensive care for patients.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Educate patients and their families on how to continue their progress and maintain their independence after leaving our facility.

Overview

Job Title: Occupational Therapist

Location: Norfolk, VA

Salary: $39-44 per hour

Schedule: 35 hours per week, Monday-Friday, no weekend rotation

Facility: 220-bed Skilled Nursing Facility and Long Term Care facility
