Job summary

Hands of Hope Physical Therapy in the Bronx is seeking a dedicated Occupational Therapist (OT) to join our outpatient team in Morris Park. You will evaluate patients, design individualized plans, and deliver expert OT interventions to help clients regain independence.

Qualifications

• Active New York OT license.
• Graduate of an accredited OT program.
• Excellent clinical reasoning and communication skills.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Conduct comprehensive OT evaluations and develop individualized treatment plans.
• Deliver skilled OT interventions to improve ADLs, coordination, strength and cognition.
• Educate patients and caregivers on home exercise programs and adaptive equipment.
• Maintain compliant EMR documentation and monitor patient progress.
• Collaborate with physicians and the interdisciplinary team to ensure coordinated care.

What You'll Do Day-to-Day

As an Occupational Therapist at Hands of Hope, you will:

• Evaluate & Plan: Perform comprehensive OT evaluations based on physician referrals and develop individualized treatment plans tailored to each patient's clinical needs and functional goals.
• Deliver Skilled Interventions: Provide therapeutic interventions to enhance strength, coordination, fine motor skills, cognitive function, and activities of daily living (ADLs).
• Guide & Educate: Teach patients and caregivers safe home exercise programs, adaptive equipment usage, and self-management strategies to maximize independence.
• Track & Refine: Monitor patient progress, modify treatment plans as appropriate, and maintain accurate, timely, and compliant EMR documentation.
• Collaborate for Better Outcomes: Work closely alongside physicians, physical therapists, and interdisciplinary team members to deliver seamless, coordinated care while maintaining HIPAA compliance and professional standards.

What We're Looking For

Required:

• Active Occupational Therapist (OT) License in the State of New York.
• Graduate of an accredited Occupational Therapy program.
• Excellent clinical reasoning, communication, and interpersonal skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.

Preferred:

• Previous experience in an outpatient rehabilitation or therapy clinic.
• Experience treating orthopedic, neurological, and post-surgical patients.
