Job summary

Homeside Rehab in New York is seeking compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to deliver in-home therapy across the Bronx. You will assess, treat, and empower patients to regain independence in daily activities.

Youll work with an interdisciplinary team, educate families, and document progress in electronic records, with flexible hours and a supportive environment. A NY license and valid driver's license are required.

Qualifications

• Active New York State OT or COTA license (Required)
• Minimum 1 year of Home Health, Outpatient, Early Intervention, or SNF experience preferred
• Excellent clinical assessment and communication skills
• Ability to work independently in a home health setting
• Basic computer proficiency for electronic documentation
• Valid New York State driver’s license and reliable transportation

Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized treatment plans based on patient needs and goals
• Provide evidence-based interventions to improve fine motor skills, upper extremity function, ADLs, sensory processing, coordination, and independence
• Deliver skilled therapy services in patients' homes
• Complete accurate, timely, and compliant electronic documentation
• Collaborate with physicians, caregivers, and the interdisciplinary team to coordinate care
• Educate patients and caregivers on home exercise programs, adaptive equipment, and strategies
• Communicate scheduling and clinical needs with office staff

Homeside Rehab is seeking compassionate and dedicated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide individualized, evidence-based occupational therapy services to patients in their homes throughout the Bronx.

Responsibilities

• Perform comprehensive occupational therapy evaluations (OTs) and develop individualized treatment plans based on each patient's needs and goals.

• Provide evidence-based occupational therapy interventions to improve fine motor skills, upper extremity function, activities of daily living (ADLs), sensory processing, coordination, and functional independence.

• Deliver skilled therapy services in patients' homes while promoting improved function and quality of life.

• Complete accurate, timely, and compliant electronic documentation.

• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.

• Educate patients and caregivers on home exercise programs, adaptive equipment, and therapeutic strategies.

• Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.

Qualifications

• Active New York State Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) License (Required)

• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred

• Excellent clinical assessment and communication skills

• Ability to work independently in a home health setting

• Basic computer proficiency for electronic documentation

• Valid New York State driver's license and reliable transportation

Position Details

• Job Types: Full-time, Part-time, Per Diem

• Compensation: $75.00–$100.00 per hour

Schedule

• Choose your own hours

• Monday through Friday

• Day and evening shifts available

• Weekend availability optional
