Job summary

Qualifications

We are hiring on behalf of our client, seeking compassionate and motivated Occupational Therapists to join our growing home health team throughout the New York metropolitan area. Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized, goal-oriented treatment plans.
• Provide skilled occupational therapy services directly in patients' homes to improve independence with Activities of Daily Living (ADLs) and Instrumental ADLs (IADLs).
• Treat both pediatric and adult patients, addressing short-term rehabilitation needs (e.g., post-stroke, post-surgery) and long-term developmental or chronic care needs.
• Assess home environments and recommend adaptive equipment, assistive technology, and safety modifications.
• Collaborate closely with physicians, physical therapists, speech-language pathologists, nurses, caregivers, and interdisciplinary team members.
• Complete timely, accurate clinical documentation, progress notes, and care plans.
• Educate patients, families, and caregivers on energy conservation techniques, fine/gross motor exercises, safety, and functional recovery goals.
