Job summary

Premier Home Health Providers Inc. seeks an Occupational Therapist to support patients at home, evaluate home environments, and design personalized treatment plans to improve independence and safety.

The role involves supervising OT Assistants, coordinating with the care team, documenting care, and guiding families. Per diem position paying $130 per visit; travel to patient homes across California required.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited jointly by the association
• Registered with the NBCOT (National Board for Certification in OT)
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• CPR certification with AHA that is current
• Valid driver’s license and reliable insured vehicle with ability to travel to patient homes
• Strong communication and organizational skills

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient's treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
