Job summary

Arbor Rehab & Nursing – Lodi, CA is seeking a full-time Occupational Therapist to screen, evaluate, and treat patients with cognitive and ADL impairments. You will develop plans of care and document progress following clinical standards.

Compensation includes $60–$67 per hour based on experience and a $5,000 signing bonus payable in 5 months. Open to applicants at all experience levels with benefits and growth opportunities.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop a plan of care.
• Use standardized assessments and evidence-based practice to support interventions.

• Adjust treatments based on patient progress and documentation in the clinical record.
• Communicate with supervisor and health team members regarding patient progress and plans.
• Provide discharge summaries per regulatory and clinical requirements.

Key Responsibilities

• Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Adjust services in relation to patient evaluation and refer as appropriate.
• Provide consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
