Interim HealthCare of Northern Indiana is seeking a Home Health Occupational Therapist to help patients regain independence in their own homes. You will assess, treat, and educate within a patient-centered plan of care, collaborating with a multidisciplinary team.

Requirements include an accredited OT degree, Indiana OT license, CPR, and at least 1 year of OT experience in home health. Competitive salary plus benefits and growth opportunities.

• Graduate of an accredited Occupational Therapy Program.
• Active Indiana OT license.
• CPR certification.
• Minimum 1 year OT experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.
• Good clinical judgement and compassionate demeanor.

Responsabilidades

• Provide occupational therapy to patients unable to perform daily tasks at home.
• Work with a home health team to implement the plan of care and goals.
• Assess patients, set therapy goals, and document progress.
• Assist with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment to enhance safety and independence.
• Educate patient and family on plan, exercises and self-care.
• Assess fall risks and promote home safety.
