Qualifications

  • Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility.
  • Knowledge of current occupational therapy practice standards and regulations.
  • Proficient in using electronic medical records and other healthcare software.

Responsibilities

  • Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments.
  • Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
  • Provide skilled occupational therapy interventions, including therapeutic exercise, activities of daily living training, cognitive retraining, and environmental modifications.
  • Monitor and document resident progress and adjust treatment plans as necessary.
  • Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
  • Educate residents and family members on occupational therapy goals, treatment plans, and home exercise programs.
  • Maintain accurate and timely documentation of occupational therapy services provided.
  • Participate in team meetings, in-service education, and quality improvement initiatives.
  • Maintain current knowledge of occupational therapy practice and relevant regulations.

Job description
