Job summary

Angels Care Home Health is seeking an Occupational Therapist to assess patients, develop and update plans of care, and coordinate therapy services in a home health setting in Virginia. The role requires licensure and experience to determine patient needs and treatment targets.

The OT will carry out evaluations, document progress, and adapt plans using infection control and universal precautions while considering the patient’s physical, social, and cultural environment for optimal home-based

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of patient.
• Prepare, complete a Plan of Care, progress notes, and other clinical documentation on an ongoing basis.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in coordinating/performing patient care.
• Assess physical, social, and cultural components of the patient’s occupational environment.
