CommonSpirit Health at Home is seeking a skilled Occupational Therapist to join the VNA Health at Home team, providing skilled therapy in patients' homes. The OT will assess, set goals and develop a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

QUALIFICATIONS include a Bachelor's degree in OT, an active OT license, CPR/BLS certification, and at least one year of OT experience, plus a current driver's license.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active OT license in the state where you practice.
• Current CPR/BLS Certification.
• One-year Occupational Therapy experience in a clinical setting.
• Current Drivers License.

Responsabilités

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized treatment plans.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities.
• Maintain accurate and timely records of patient progress and treatment plans for compliance.
• Monitor patient progress and adjust treatment plans as necessary to meet changing needs.
• Collaborate and communicate patient changes with physicians, nurses, therapists and other home health team members.

Job Summary and Responsibilities

We are seeking a skilled and compassionate Occupational Therapist to join the VNA Health at Home team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

• Active registered OT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year Occupational Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations
