Job summary

Wedgewood Healthcare and Rehabilitation in Lakeland, FL seeks a compassionate Occupational Therapist to empower residents to regain independence through customized therapy plans and collaborative team care.

You will evaluate, plan, implement, and document progress, working with a multidisciplinary team to deliver evidence-based care and uphold confidentiality.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Florida Occupational Therapist license.
• Experience in long-term care or skilled nursing facilities preferred.

Responsibilities

• Conduct evaluations and develop individualized treatment plans tailored to residents’ functional goals.
• Implement engaging, evidence-based interventions to promote independence in activities of daily living.
• Monitor and document residents’ progress, adjusting care plans as needed.
• Collaborate with a multidisciplinary team to ensure comprehensive care.
• Support the rehabilitation of residents recovering from illness, injury, or disability.
• Maintain accurate, timely documentation and safeguard resident confidentiality.
