Heritage Manor Care in Lake Forest, California, is seeking an Occupational Therapist to provide high-quality patient care in a skilled nursing facility. The role involves direct patient assessments, treatment, and care planning within a collaborative team.

You will screen and evaluate patients, develop individualized treatment plans, educate families, and document progress while supervising OT assistants and ensuring compliance with state licensing and NBCOT requirements.

Qualifications

• Bachelor's degree in Occupational Therapy.
• Licensed as required by State; NBCOT registered.
• CPR certification is required.
