Heritage Manor Care in Valley Center, CA, is seeking a dedicated Occupational Therapy Assistant to join our post-acute care team. You will implement therapy plans under the supervision of an OTR, assist with patient progress notes, and collaborate with staff to support recovery goals.

The ideal candidate has an OTA degree and state licensure, CPR certification, and 1–2 years of experience in a residential or skilled setting, or recent graduates will be considered for entry into our rehab

Qualifications

  • Associate of Arts in Occupational Therapy Assistant is required.
  • Licensure or certification as required by state guidelines.
  • CPR certification is required.
  • 1–2 years OTA experience preferred or graduates considered.

Responsibilities

  • Provides therapy services per the OT Plan of Care.
  • Works under supervision of an OTR per guidelines.
  • Communicates changes in treatment to the OTR.
  • Completes daily notes and aids in progress and discharge reports.
  • Handles billing, payroll and expense documentation timely.
  • Communicates with patients, families and referring physicians.
  • Participates in continuing education to stay current.
  • Coordinates treatment goals with the care team.
  • Maintains licensure and state certifications as required.

Responsibilities
  • Provides Therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
  • Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
  • Consults with OTR regarding changes in treatment or condition in a timely manner.
  • Appropriately and effectively completes documentation including daily notes and assisting the OTR to create progress notes, discharge summaries, or monthly reports as needed.
  • Submits billing, payroll, and expense documentation in a timely manner.
  • Communicates with patients, families, referring physicians and other members of the team.
  • Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
  • Participates in care planning meetings, rehab meetings and family training in order to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
  • Follows organizational and facility policies and procedures to ensure employee and patient safety.
  • Ensures equipment is in good working order prior to providing care to patients. Follows procedures for reporting damaged equipment.
  • Maintains a clean environment.
  • Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
  • Coordinates treatment goals with other team members.
  • Maintains valid licensure, certification or equivalent as requested by appropriate state agency.
Education
  • Minimum of Associate of Arts in Occupational Therapy Assistant
  • Licensure/Certification: Licensure as required by State guideline
  • Certification as required by State
  • CPR Certification
Experience
  • 1 year Occupational Therapy Assistant experience preferred.
  • 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
  • Graduates in Occupational Therapy Assistant Program will be considered.
