Job summary

Heritage Manor Care administers occupational therapy services in a post-acute care setting, seeking a dedicated OTA to work under supervision of an OTR and in accordance with state/federal guidelines.

The role includes documenting daily notes, assisting with progress notes and reports, coordinating with the rehab team, and staying current with licensure and CPR requirements. Excellent communication with patients and families is essential.

Qualifications

• Associate of Arts in Occupational Therapy Assistant is required.
• Licensure as required by State guidelines.
• CPR certification is required.

Responsibilities

• Provides therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
• Provides therapy services under supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with OTR regarding changes in treatment or condition in a timely manner.
• Completes documentation including daily notes and assists the OTR to create progress notes, discharge summaries, or monthly reports as needed.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, referring physicians and other team members.
• Participates in continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Job Duties

• Provides Therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
• Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with OTR regarding changes in treatment or condition in a timely manner.
• Appropriately and effectively completes documentation including daily notes and assisting the OTR to create progress notes, discharge summaries, or monthly reports as needed.
• Submits billing, payroll, and expense documentation in a timely manner.

• Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Participates in care planning meetings, rehab meetings and family training in order to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
• Follows organizational and facility policies and procedures to ensure employee and patient safety.
• Ensures equipment is in good working order prior to providing care to patients. Follows procedures for reporting damaged equipment.
• Maintains a clean environment.
• Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Required

Education Required:

• Minimum of Associate of Arts in Occupational Therapy Assistant
• Licensure/Certification: Licensure as required by State guideline
• Certification as required by State
• CPR Certification

Experience

• 1 year Occupational Therapy Assistant experience preferred.
• 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
• Graduates in Occupational Therapy Assistant Program will be considered.
