Job summary

Heritage Manor Care in Garden Grove, CA is seeking a dedicated Occupational Therapy Assistant to support therapy plans under an OTR. The role focuses on delivering patient-centered care in a post-acute setting.

You will document progress, collaborate with a multi-disciplinary team, and participate in ongoing training to maintain licensure and improve patient outcomes. Strong communication and adherence to guidelines are essential.

Qualifications

• Associate degree as Occupational Therapy Assistant (OTA) required.
• Licensure or certification as required by state guidelines.
• CPR certification required.

Responsibilities

• Provides therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
• Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with OTR regarding changes in treatment or condition in a timely manner.
• Completes documentation including daily notes and assisting the OTR to create progress notes, discharge summaries, or monthly reports as needed.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, referring physicians and other members of the team.
• Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Participates in care planning meetings, rehab meetings and family training to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
• Follows organizational and facility policies and procedures to ensure employee and patient safety.
• Ensures equipment is in good working order prior to providing care to patients. Follows procedures for reporting damaged equipment.
• Maintains a clean environment.
• Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Education

• Minimum of Associate of Arts in Occupational Therapy Assistant
• Licensure/Certification: Licensure as required by State guideline
• Certification as required by State
• CPR Certification

Experience

• 1 year Occupational Therapy Assistant experience preferred.
• 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
• Graduates in Occupational Therapy Assistant Program will be considered.
