Job summary

LiveWell Homecare Agency in Georgia is seeking an Occupational Therapist to deliver age-appropriate rehabilitative interventions in a home health setting. You will assess patients, develop care plans with the care team, and provide direct therapeutic exercises.

The role requires OT licensure and BLS, with at least one year of full-time experience (home health preferred). You will educate patients and caregivers, document progress, and coordinate services to promote independence and safe recovery.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Valid Occupational Therapist license and BLS certification.
• Minimum of one year full-time occupational therapy experience; home health experience preferred.
• Valid driver’s license and private vehicle insurance.
