Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• Experience in SNF or long-term care settings preferred.
• Knowledge of current OT treatment practices.
• Familiarity with PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize ADL/IADL safety and performance.
• Ensure treatment aligns with the established plan of care.
• Monitor patient response and adjust approach with IDT collaboration.
• Complete timely documentation meeting regulatory requirements.

Position Overview:

Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.

Key Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
