James River Home Health and Hospice in Charlottesville, VA is seeking a full-time Occupational Therapist to deliver skilled home health OT and collaborate with the interdisciplinary team for optimal patient care.

The role offers a salary of $88,000–$117,000 based on experience, with flexible schedules, travel allowances, company-paid devices, generous PTO, comprehensive benefits, a 401K match, and career advancement opportunities.

Competenze

• Must be a graduate of an approved Occupational Therapy school.
• Must be licensed in Virginia.
• Must have at least one year of institutional experience.

Home Health Occupational Therapist

You will be responsible for providing skilled Occupational Therapy to clients in their homes on an intermittent basis. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Salary: $88,000 - $117,000 (Based on Experience)

Brief overview of Responsibilities:

• Assist the physician in evaluating the client’s level of function by applying diagnostic and prognostic procedures


• Conduct ongoing assessments of the client’s needs, including outcome and assessment information set (OASIS) assessments


• Evaluate the client’s significant other(s), and the home situation to determine what instruction will be required and what family assistance will be available caring for the client, as well as what other agency and community services will be required


• Assist in development and implementation of the interdisciplinary client care plan to include Occupational Therapy in consultation with the physician


• Teach, supervise and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home.


• Confirm, on a weekly basis, the scheduling of visits with the Director of Therapy in order to coordinate agency visits


• Prepare clinical/progress notes on the day of the visit which are incorporated into the chart within 24 hours.


• Other duties as assigned

Qualifications:

• Must be a graduate of an approved Occupational Therapy school


• Must be licensed in Virginia


• Must have at least one year of institutional experience
