Job summary

James River Home Health and Hospice in Lynchburg, VA is seeking a compassionate Full-Time Occupational Therapist to join our locally owned team. You will provide skilled OT services in patients' homes on an intermittent basis and collaborate with an interdisciplinary team to ensure quality care.

Salary ranges from $83,700 to $117,000 annually. The position offers generous time-off, health benefits, disability and life insurance, a 401K with employer match, and opportunities for career

Qualifications

• Must be a graduate of an approved Occupational Therapy school.
• Must be licensed in Virginia.
• Must have at least one year of institutional experience.

Responsibilities

• Assist the physician in evaluating the client’s level of function by applying diagnostic and prognostic procedures
• Conduct ongoing assessments of the client’s needs, including OASIS assessments
• Evaluate the client’s home situation and family support to determine required instruction and community services
• Develop and implement the interdisciplinary client care plan including Occupational Therapy in consultation with the physician
• Teach, supervise, and counsel the family and client in the total Occupational Therapy program at home
• Confirm scheduling of visits with the Director of Therapy weekly
• Prepare clinical/progress notes on the day of the visit and document within 24 hours
• Other duties as assigned

A brief overview of Responsibilities:

• Assist the physician in evaluating the client’s level of function by applying diagnostic and prognostic procedures
• Conduct ongoing assessments of the client’s needs, including outcome and assessment information set (OASIS) assessments
• Evaluate the client’s significant other(s), and the home situation to determine what instruction will be required and what family assistance will be available to care for the client, as well as what other agency and community services will be required
• Assist in the development and implementation of the interdisciplinary client care plan to include Occupational Therapy in consultation with the physician
• Teach, supervise, and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home.
• Confirm, every week, the scheduling of visits with the Director of Therapy to coordinate agency visits
• Prepare clinical/progress notes on the day of the visit and incorporate them into the chart within 24 hours.
• Other duties as assigned
