James River Home Health and Hospice in Fredericksburg, VA is seeking a PRN Occupational Therapist to provide skilled OT services in patients' homes on an as-needed basis. You will perform assessments, develop care plans, and coordinate with the interdisciplinary team to support functional independence.

Responsibilities include ongoing patient assessments, OASIS-related activities, educating families, documenting progress within 24 hours, and scheduling visits with therapy leadership.

Qualificações

• Must be a graduate of an approved Occupational Therapy school.
• Must be licensed in Virginia.
• Must have at least one year of institutional experience.

Responsabilidades

• Assist the physician in evaluating client function and prognosis.
• Conduct ongoing assessments including OASIS-related data.
• Assess home situation and coordinate family involvement and community resources.
• Develop and implement the interdisciplinary OT care plan with the physician.
• Teach, supervise, and counsel the family and patient in the OT program at home.
• Confirm weekly visit scheduling with the Director of Therapy.
• Prepare progress notes within 24 hours of visits.
• Perform other duties as assigned.

You will be responsible for providing skilled Occupational Therapy to clients in their homes intermittently. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient.

Compensation: $38.70 - $48.00 per hour

A brief overview of Responsibilities:

• Assist the physician in evaluating the client’s level of function by applying diagnostic and prognostic procedures
• Conduct ongoing assessments of the client’s needs, including outcome and assessment information set (OASIS) assessments
• Evaluate the client’s significant other(s), and the home situation to determine what instruction will be required and what family assistance will be available to care for the client, as well as what other agency and community services will be required
• Assist in the development and implementation of the interdisciplinary client care plan to include Occupational Therapy in consultation with the physician
• Teach, supervise, and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home.
• Confirm, every week, the scheduling of visits with the Director of Therapy to coordinate agency visits
• Prepare clinical/progress notes on the day of the visit which are incorporated into the chart within 24 hours.
• Other duties as assigned
