Occupational Therapist (OT) – Per Diem
Job Summary
Prestige Care Center of Fairfield is seeking a compassionate, skilled, and dedicated Occupational Therapist (OT) to join our rehabilitation team on a per diem basis. The Occupational Therapist will provide high-quality, patient-centered care to individuals working to improve functional independence, activities of daily living (ADLs), strength, coordination, cognition, safety, and overall quality of life.

About the Role
As an Occupational Therapist, you will evaluate patients' functional abilities, develop individualized treatment plans, and provide skilled therapeutic interventions designed to maximize independence and participation in meaningful daily activities.
Essential Responsibilities
- Evaluate patients' functional, physical, cognitive, and psychosocial abilities and identify individualized occupational therapy needs.
- Develop, implement, and modify individualized treatment plans based on patient goals, clinical findings, functional limitations, and response to treatment.
- Provide skilled occupational therapy services, including ADL and IADL training, therapeutic activities, therapeutic exercises, neuromuscular re-education, functional mobility training, balance activities, transfer training, and other appropriate interventions.
- Address areas such as dressing, bathing, grooming, toileting, feeding, functional transfers, upper-extremity function, coordination, and activity tolerance to promote greater independence.
- Assess and address cognitive, perceptual, and safety-related deficits that may impact functional performance and independence.
- Recommend and provide training in adaptive equipment, assistive devices, compensatory techniques, and environmental modifications as clinically appropriate.
- Monitor patient progress and adjust treatment approaches and goals based on clinical findings and patient response.
- Educate patients, families, and caregivers regarding treatment programs, home exercise and activity programs, adaptive techniques, fall prevention, energy conservation, joint protection, and safety strategies.
- Communicate effectively with patients, families, caregivers, physicians, nursing staff, and other members of the interdisciplinary team.
- Collaborate with Physical Therapy, Speech Therapy, Nursing, and other healthcare professionals to support comprehensive, patient-centered care.
- Maintain accurate, timely, and complete documentation of evaluations, treatment sessions, progress, recommendations, and discharge planning in accordance with facility and regulatory requirements.
- Participate in care planning and interdisciplinary meetings as appropriate.
- Promote patient safety and follow all facility policies, procedures, infection-control practices, and applicable state and federal regulations.
- Stay current with evidence-based practices, clinical guidelines, and developments in occupational therapy to support high-quality patient outcomes.
- Maintain professional standards and protect patient privacy and confidentiality.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current and unrestricted Occupational Therapist license in the applicable state.
- Current CPR certification, as required by facility policy.
- Previous experience in skilled nursing, rehabilitation, long-term care, or geriatric care is preferred but not required.
- Strong clinical, communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
