Job summary

Prestige Care Center of Fairfield is seeking an Occupational Therapist (OT) on a per diem basis to support our rehabilitation team. The OT will evaluate, treat, and educate patients to improve independence in ADLs and IADLs while collaborating with other therapy professionals.

The role emphasizes patient-centered care, safety, and adherence to facility policies. Per diem schedule provides flexible coverage based on census and needs.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and unrestricted OT license in Iowa.
• Strong clinical, communication, and interpersonal skills.

Responsibilities

• Evaluate patients' functional, physical, and cognitive abilities and identify OT needs.
• Develop, implement, and modify individualized treatment plans to maximize independence.
• Provide ADL and IADL training, therapeutic activities, exercises, and safety strategies.
• Educate patients and families on treatment programs and home exercise.
• Maintain accurate documentation and participate in interdisciplinary care planning.
