Job summary

Sweetwater Care and Rehab is seeking an Occupational Therapist to join our in-house therapy team. The role focuses on rehabilitation for patients with cognitive and/or ADL functioning disorders, delivering evidence-based interventions and documenting outcomes.

The OT will supervise supportive staff and collaborate with the Director of Rehabilitation Services to ensure regulatory compliance and quality care.

Qualifications

• Current state licensure to practice Occupational Therapy.
• NBCOT / AOTA adherence per professional practice acts.

Responsibilities

• Provide rehabilitative therapy to patients with cognitive and/or ADL functioning disorders.
• Screen/evaluate patients and develop plans of care.
• Utilize standardized assessments and evidence-based practices.
• Deliver rehabilitative, skilled and medically necessary treatment per regulatory standards.
• Document treatment minutes and progress in the electronic record.
• Coordinate discharge summaries and communicate with supervisor and health team.

Principal Responsibilities

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

• Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
• Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapist.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
