Job summary

Synchrony Rehab at Liberty Heights (SLC) is seeking an Occupational Therapist to provide OT services including assessment, treatment planning, and therapeutic interventions in a collaborative, interdisciplinary setting in Colorado Springs.

Responsibilities include conducting assessments, developing individualized plans, educating patients and families, documenting progress, and coordinating with a diverse care team to ensure comprehensive patient outcomes.

Qualifications

• Must have OT degree from an accredited program.
• Experience: 0-1 years.
• Licenses and Certifications: OT licensure/registration; CPR certification preferred.

Responsibilities

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions to improve patients' functional abilities.
• Educate patients and families on daily living techniques.
• Document patient progress and adapt treatment plans as needed.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
