Vantage Health Partners is seeking an Occupational Therapist to join our Home Health/Assisted Living Facilities team covering Longmont/Loveland area. This full or part time, remote based position offers an opportunity to help patients regain independence and improve daily living quality within the Hospital/Health Care industry.

As an Occupational Therapist, you will evaluate physical, cognitive, and functional abilities, design treatment plans, and guide patients through activities that support

Qualifications

• Accredited OT degree and Colorado licensure or eligibility.
• NBCOT certification in good standing.
• Experience with EHR and clinical documentation.

Responsibilities

• Perform comprehensive OT assessments to evaluate physical, cognitive, and functional capabilities.
• Create and adjust individualized treatment plans based on patient goals and clinical findings.
• Guide patients through therapeutic activities to improve daily living skills and independence.
• Educate patients and caregivers on adaptive techniques, devices, and home modification strategies.
• Document evaluations, progress notes, and outcomes per regulatory standards.
• Collaborate with physicians, nurses, and other healthcare providers to coordinate patient care.

As an Occupational Therapist, you will evaluate patients' physical, cognitive, and functional abilities, design individualized treatment plans, and guide patients through therapeutic activities that support recovery, rehabilitation, and long-term wellness. You will collaborate closely with physicians, nurses, and other healthcare professionals to deliver coordinated, patient-centered care. This clinical role provides a supportive environment for professional development and clinical growth. Compensation is offered pay per visit or salaried option, with details shared during the hiring process.

Responsibilities

• Conduct comprehensive occupational therapy assessments to evaluate patients' physical, cognitive, and functional capabilities
• Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings
• Guide patients through therapeutic activities that improve fine motor skills, strength, coordination, and independence in daily living tasks
• Train patients and caregivers in adaptive techniques, assistive devices, and home modification strategies
• Document patient evaluations, progress notes, and outcomes accurately and in compliance with regulatory standards
• Collaborate with physicians, nurses, physical therapists, and other healthcare providers to coordinate patient care
• Educate patients and families on injury prevention, ergonomics, and energy conservation strategies
• Monitor patient progress and adjust interventions to support measurable functional improvements
• Participate in interdisciplinary team meetings and discharge planning
• Maintain up-to‑date knowledge of evidence-based occupational therapy practices and clinical guidelines

Requirements

• Degree in Occupational Therapy from an accredited program
• Current licensure as an Occupational Therapist in the state of Colorado or eligibility to obtain licensure
• NBCOT certification (Occupational Therapist Registered, OTR) in good standing
• Strong knowledge of occupational therapy assessment tools and evidence-based treatment interventions
• Proficiency with electronic health record (EHR) systems and clinical documentation
• Excellent communication and interpersonal skills for working with patients, families, and care teams
• Strong clinical reasoning, critical thinking, and problem‑solving abilities
• Compassionate, patient‑centered approach with a commitment to quality care
• Ability to work effectively in a fast‑paced, collaborative healthcare environment
• Basic Life Support (BLS) certification preferred
