Job summary

ST RAPHAELS HOME CARE SERVICES is seeking a compassionate Occupational Therapist to deliver one-on-one home health services to veterans. You will assess, plan, and implement care to improve safety, independence, and daily living activities, with a focus on ADLs, mobility, and fall prevention.

Low productivity expectations and flexible scheduling support work-life balance, plus mileage reimbursement for travel. Licensure, driving eligibility, and a passion for veterans’ well‑being are essential.

Qualifications

• Current Occupational Therapist license in the state of practice.
• Graduate of an accredited Occupational Therapy program.
• Valid driver's license and reliable transportation.
• Home health experience is a plus, but new clinicians are welcome to apply.

Responsibilities

• Perform occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans to improve safety, independence, and quality of life.
• Provide interventions to enhance ADLs, mobility, home safety, and fall prevention.
• Recommend adaptive equipment and home modifications.
• Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team.
• Complete timely, accurate documentation in accordance with home health standards.
