Vierra Communities, St Elizabeth Rehab & Nursing, is seeking a Certified Occupational Therapist to join our in-house team in a skilled nursing setting. This role covers inpatient rehab with a focus on geriatric care and functional rehabilitation.

Qualifications

• Current state licensure as an Occupational Therapist.
• Experience with long-term care/geriatric settings preferred.
• Must pass a criminal background check.
• Strong communication and interpersonal skills.
• Patience, empathy, and compassion for elderly patients.
• Ability to work cooperatively with other healthcare professionals.

Responsibilities

• Provide direct patient care and assess functional abilities for rehabilitation.
• Administer treatment plans under physician direction.
• Document patient condition and educate patients and families.
• Educate on strategies to improve activities of daily living and health.
• Collaborate with physicians, nurses, and other professionals for coordinated care.
• Participate in patient care meetings to discuss best practices and outcomes.

Required Qualifications

Major Duties and Responsibilities

• Proved direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Educate patients and their families on strategies to improve daily living skills and promote overall health.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
• Participate in patient care meetings and contribute to team discussions on best practices and treatment outcomes.
