Job summary

Apple Valley Care Center is seeking a licensed Occupational Therapist to join our skilled nursing facility team. You will evaluate and treat residents with physical, cognitive, and psychosocial challenges, helping them achieve independence and improve quality of life.

Responsibilities include assessments, personalized treatment plans, therapy interventions, progress documentation, and collaboration with an interdisciplinary team to optimize resident outcomes.

Qualifications

  • Must possess a current license to practice as an Occupational Therapist in California.
  • At least 2 years of experience in a healthcare environment, preferably within a skilled nursing facility.
  • Familiarity with current occupational therapy practice standards and regulations.
  • Proficient in the use of electronic medical records and other healthcare software.

Responsibilities

  • Assess residents' physical, cognitive, and psychosocial abilities and functional limitations through standardized assessments.
  • Create and implement personalized treatment plans based on assessment findings, resident goals, and physician directives.
  • Deliver skilled occupational therapy interventions, such as therapeutic exercises, daily living skills training, cognitive retraining, and environmental adjustments.
  • Monitor and document resident progress, making necessary adjustments to treatment plans.
  • Collaborate with interdisciplinary team members, including physicians, nurses, physical therapists, and speech therapists, to coordinate care and optimize resident outcomes.
  • Educate residents and their families on occupational therapy objectives, treatment plans, and home exercise routines.
  • Maintain accurate and timely documentation of the occupational therapy services provided.
  • Participate in team meetings, in-service training, and quality improvement efforts.
  • Stay updated on occupational therapy practices and relevant regulations.
