Stonehenge American Fork is seeking an Occupational Therapist for a full-time role in American Fork, UT. You will screen and treat patients with cognitive and ADL impairments, develop and adjust plans of care, and collaborate with a multidisciplinary team to support rehabilitation goals.

You will provide patient and family counseling, document progress per regulatory standards, and participate in program development and mentorship initiatives to promote growth and leadership within the team.

Qualifications

• Licensed Occupational Therapist credential required.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop a care plan.
• Utilize standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory and clinical requirements.
• Administer ADL retraining, therapeutic activities, exercises, modalities, and use of devices as appropriate.
• Assess patient progress and adjust treatments in the clinical record per regulatory standards.
• Evaluate treatment outcomes and refer as needed based on results.
• Provide counseling to patients and families, and coordinate with caregivers and other providers.
• Communicate effectively with supervisor and health team about progress and plans.
• Prepare comprehensive discharge summaries per regulatory requirements.

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
