Concentra in Milpitas, CA is seeking a Physical Therapist to perform supervisory duties and evaluations, helping patients improve motor function and daily living tasks. You will collaborate with medical staff and educate patients while maintaining high standards of care.

The role requires a Bachelor’s in Physical Therapy, with a Master’s preferred, and emphasizes HIPAA compliance, excellent communication, and patient-centered outcomes. Compensation ranges $60–$70 hourly.

Qualifications

• Bachelor’s Degree from an accredited Physical Therapy program.
• Masters’ Degree from an accredited Physical Therapy program, preferred.

Responsibilities

• Ensure delivery of high quality patient care services.
• Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems.
• Support and encourage multidisciplinary coordination of treatment.
• Ensure proper documentation of patient care.
• Maintain professional standards through compliance with quality assurance policies and procedures and applicable regulations.
• Implement effective communication with own and other departments' staff, medical staff and patients for coordinated high quality care.
• Demonstrate effective problem-solving and conflict-resolution abilities.
• Inform Administrator of department problems, needs and solutions in a timely manner.
• Plan, design, implement and interpret specific physical therapy programs for recovery.
• Educate patients in proper care and use of supports and exercise programs.
• Perform or delegate treatment including modalities, exercises, gait training and hydrotherapy.
• Follow documentation and reporting procedures.
• Secure prescription for each patient treated.
• Work closely with the physician to ensure comprehensive service delivery.
