Job summary

SGS Consulting seeks an experienced registered nurse to provide competent nursing care, triage, and education for chronic conditions in a dynamic clinical setting. You will respond to calls, manage medications, and collaborate with care teams to implement care plans for patients across multiple panels.

The role emphasizes patient-centered care, safety, and excellent service delivery, with the opportunity to work in person or via telehealth and to support preventative care and community health

Qualifications

• Active RN license (Louisiana or compact); BLS certification is required.
• Experience in patient education and chronic disease management.
• Proficiency in EHR navigation and computer use.

Responsibilities

• Provide competent nursing care and execute responsibilities safely and consistently.
• Respond to telephonic requests promptly and ensure high responsiveness.
• Provide triage and symptom management to patients whether on-site or remotely.
• Utilize standardized protocols for medication management, refills, and prior authorizations.
• Review patient medications thoroughly and update when needed.
• Educate patients and provide direct care, including chronic disease management, in person or remotely.
• Collaborate with care teams to monitor care plan progress for patients on multiple panels.
• Build relationships with patients and caregivers to support preventive care and ED/hospital diversion.
• Create a welcoming environment for patients, communities, families, and teams.
• Delegate activities to clinical staff to support patient needs.
• Participate in care team meetings to discuss patient care and clinic operations.
• Deliver excellent patient experience through service, responsiveness, and respectful care.
• Perform point-of-care testing, procedures, and phlebotomy as needed.
• Performs other related duties as assigned.
