Thomas Jefferson University Hospitals, Inc. is seeking an Obstetrics Nurse Navigator to provide individualized clinical navigation, education, and care coordination across the obstetric ambulatory care continuum.

The role acts as a central point of contact for patients, families, and care teams to support timely access and clear communication. The Nurse Navigator will address barriers to care, coordinate referrals and tests, and support patients through pregnancy and postpartum pathways,

Qualifications

• Bachelor’s Degree in Nursing required.

Responsibilities

• Provides clinical navigation and nursing support to obstetric patients during office visits, calls, and messages.
• Coordinates referrals, testing, procedures, and follow-up with providers and departments.
• Educates patients and families about diagnoses, treatment plans, tests, referrals, and next steps.
• Identifies barriers to care and connects patients with resources to facilitate timely access and continuity of care.
• Tracks patient follow-up and ensures necessary records and results are available for care.
