Job summary

Thomas Jefferson University Hospitals, Inc. is seeking an Obstetrics Nurse Navigator to provide individualized clinical navigation, patient education, and care coordination for obstetric patients across the ambulatory continuum.

This role serves as a central contact for patients, families, and providers to ensure timely access to care and clear communication. The RN role requires a Bachelor of Science in Nursing and at least 3 years of obstetric/ambulatory nursing experience, with Pennsylvania

Qualifications

• BSN required.
• Minimum of 3 years nursing experience in ambulatory obstetrics, maternal health, care coordination, patient navigation, or related setting.

Responsibilities

• Coordinate timely access to appointments, testing, imaging, referrals, and authorizations.
• Educate patients and families on diagnoses, treatments, and next steps.
• Provide competent clinical care coordination and nursing support during visits, calls, and messages.
• Identify barriers to care and connect patients with available resources.
• Track follow-up and improve care continuity through teamwork.
