Job summary

Valley Community Services Board is seeking an OBAT Care Coordinator to support individuals with opioid and/or alcohol use disorders. You’ll assess needs, coordinate with treatment providers, and help clients access resources across Staunton, Waynesboro, Augusta County, and Highland County.

Qualifications

Valley Community Services Board is seeking a Care Coordinator to join our Office Based Addiction Treatment (OBAT) team. In this role, you'll work directly with individuals experiencing opioid and/or alcohol use disorders, helping them access treatment, overcome barriers to care, and connect with the resources and support they need for recovery.

You'll be part of a collaborative treatment team serving individuals and families throughout Staunton, Waynesboro, Augusta County, and Highland County. This position combines office-based services with work in the community, giving you the opportunity to build relationships and make a direct impact on the people we serve.

What You'll Do

As an OBAT Care Coordinator, you'll help individuals navigate treatment and connect with the services they need to support their recovery. Responsibilities include:

Provide ongoing substance use care coordination for individuals with opioid and/or alcohol use disorders.

Meet with individuals in office, community, and home-based settings throughout our service area.

Assess each individual's strengths, needs, goals, and barriers to recovery and develop individualized treatment plans.

Connect individuals with treatment providers, community resources, and other services that support recovery and stability.

Follow up on referrals, monitor progress, help solve barriers to care, and advocate for individuals as they navigate treatment.

Assist with substance use assessments for individuals referred to the OBAT program.

Coordinate with community agencies and treatment providers, including referrals to inpatient or residential treatment when a higher level of care is needed.

Work closely with the OBAT team through weekly staff meetings and ongoing clinical and administrative supervision.

Maintain timely, accurate documentation in accordance with Medicaid and program requirements.

Complete monthly chart audits to support accurate care coordination billing.

Requirements

Minimum Qualifications

To be considered for this position, you must:

• Have a bachelor's degree from an accredited college or university.
• Hold a Certified Substance Abuse Counselor (CSAC) credential or be eligible and willing to become a CSAC Supervisee within six months of hire. This may be satisfied by your college coursework, or provided by the agency at no cost.
• Maintain a valid Virginia driver's license in good standing, as determined by agency requirements.

Preferred Qualifications

We'd especially like to hear from candidates who bring one or more of the following:

• Bachelor's degree in Human Services or a related field.
• Experience providing addiction treatment services through aCommunity Services Board (CSB), DBHDS-licensed program, Medications for Opioid Use Disorder (MOUD) program, or another substance use treatment setting.
• Previous case management or care coordination experience.
• Familiarity with Medicaid, Virginia Department of Medical Assistance Services (DMAS) requirements, or behavioral health service workflows.
• Experience using American Society of Addiction Medicine (ASAM) criteria to help determine appropriate levels of care.
• LMHP or LMHP-E status.
• Strong organizational skills and attention to detail, including the ability to manage documentation, deadlines, and multiple client needs.
• Excellent written and verbal communication skills.
• The ability to collaborate effectively with clients, coworkers, treatment providers, and community partners.
• The ability to navigate challenges and interpersonal differences professionally and constructively.
• A demonstrated ability to work effectively with people from a variety of backgrounds and life experiences.
• A genuine interest in serving and strengthening our community.
